Early intervention in exercise rehabilitation can prevent acute pain from developing into a chronic, long-term disability. But what does this look like in practice, when a client is dealing with debilitating pain, high stress and a physically demanding job?
At the end of 2025, we were pleased to introduce the StrongStart program – a 12-week, fixed-fee, early engagement program built specifically for customers within the first 12 months post-injury or illness (the ideal referral point within 3 months). By tackling physical limitations and psychosocial barriers right out of the gate, StrongStart ensures that workers can move past the challenges that usually compromise recovery.
Dan (name changed for privacy) was one of our first referrals into the StrongStart program. This is his story – showcasing the real-world impact of a timely referral to exercise physiology.
Foundations Unsteady
Dan was a self-employed bricklayer and business owner, accustomed to working 50 hours a week. His job was intensely demanding: lifting and placing up to 600 bricks a day (4–5 kg each), carrying 25 kg concrete buckets across uneven ground and maneuvering 50 kg wheelbarrows.
In July 2025, he was diagnosed with an L5/S1 disc herniation with left-sided radicular pain. He had a two-year history of intermittent sciatica, but a severe acute flare-up left him unable to walk. His pain baseline sat at a constant 4/10, frequently spiking to an 8/10.
When our Exercise Physiologist Sidney conducted the initial assessment 2 months after his injury, Dan was in a vulnerable position:
- Physical Limitations: Trunk flexion was limited to just 45 degrees due to muscle spasms and severe movement guarding. He could only sit for 5 to 10 minutes, drive for 30 minutes, and could barely walk his dog before needing to sit down.
- Psychosocial Stressors: As the primary earner in his household and facing a cancer diagnosis within his wider family, his financial and emotional stress was immense. His sleep was broken and anxiety was high.
- Business Impact: Unable to work on-site, he was actively winding down his business and staff, believing a return to heavy manual labor was completely unrealistic.
The StrongStart Plan: Beyond the Gym Floor
Sidney’s program emphasised education, progressive exposure and lifestyle support:
1. Education & Self-Management
The foundational education focused on equipping Dan with active pain management strategies. Sidney taught Dan how to adapt his daily postures and minimize triggering positions, utilising planned micro-breaks and frequent posture variation. To break down pain guarding behaviours, Sidney helped to normalize muscle stiffness as a standard part of physical adaptation rather than a sign of ongoing damage, encouraging consistent daily mobility and structured warm-up routines before any gym or other physical activities.
2. Graded Exposure & Conditioning
Progress was driven by tolerance and symptom control, rather than a rigid timeline. Sidney promoted graded exposure to essential movement patterns – specifically bending, twisting and lifting – carefully keeping activities within Dan’s tolerances. The programming prioritized upright functional strength exercises over seated machines to foster spinal control throughout movement.
For general conditioning and active recovery, moderate-intensity cardio was introduced, with an emphasis on swimming. To keep engagement and motivation high, Sidney linked these training goals to Dan’s personal passions, building the paddling endurance, prone extension and upper-body stamina required for surf and spearfishing readiness.
3. Mental Health and Sleep Support
Addressing physical symptoms alone is never enough when a client faces high stress and anxiety. To support Dan’s emotional well-being, Sidney integrated mindfulness practices specifically targeted at mitigating anxiety. She also established a dedicated 30-to-45-minute pre-bed wind-down routine – focusing on light mobility, calming audio, and screen time reduction – to address his non-restorative, broken sleep patterns. Psychologist support was initiated to deal with some of Dan’s existing trauma.
4. Collaborative Return to Work Planning
As Dan rebuilt his capacity, Sidney provided his team with advice on how to structure a graded hours plan. This strategy mapped out a steady, predictable build toward full-time capacity, incorporating built-in recovery strategies – like strategic walking and swimming – while ensuring gym workloads remained lighter during the initial work adjustment period.
5. Communication
The StrongStart program was built with communication top of mind – delivering routine reporting and email updates to all relevant parties throughout the 12 week program.
The Outcome: Measurable, Durable Recovery
Over the course of the 12 week program, Dan worked consistently, ticking off 3-4 gym sessions per week with confidence.
Restored Mobility and Functional Strength:
- Trunk flexion improved from a heavily guarded 45 degrees, to an unrestricted 90 degrees.
- His 60-second sit-to-stand performance improved from 15 reps to 32 reps.
- Strength improved with repetitive floor-to-waist lifting up to 25kg with good technique, and carrying up to 50 kg safely.
Expanded Daily Tolerances:
- Walking endurance went from 10-minutes to a consistent 30-to-90-minutes.
- Sitting tolerance improved from 5–10 minutes to 45 minutes.
- Driving capacity increased from 30-minutes to a comfortable 60-to-90 minutes.
- Dan improved his capacity for daily activities such as mowing the lawns, completing household chores, and enjoying 30 to 45 minutes of surfing.
Rebuilding Full Work Capacity
By the end of the program, Dan had achieved full physical readiness required for the heavy manual handling demands of bricklaying. With his physical health restored and his mental health in a better place, Dan chose to pivot his career towards a role that he felt would be more sustainable long term. He gained employment in an administrative role that challenged him cognitively (occasional lifting up to 25kg required).
Dan built up his hours with the graded RTW plan and achieved a sustainable, durable return to work.
What This Means for Referrers
Dan came to us with a moderate-high risk of long term disability. By initiating the StrongStart program early, Sidney targeted his physical limitations before they became chronic, suggested strategies for improving his sleep and mental health and worked to build his confidence with movement.
